A time for thanks

Here I am sitting on my computer on Thanksgiving watching the Packers-Lions football game and the commentators made a comment about being thankful and grateful for all that they have and how lucky they are. I paused for a second and thought that I feel the same way and it would be a great time to tell everyone how thankful and lucky I am. I have a wonderful family, an amazing 4 1/2 year old son Aidan and a very supportive and understanding wife Risa who is the love of my life. 4 1/2 years ago I was fortunate enough to purchase the practice of a local dentist who passed away suddenly. Over the past few years I have assembled a great team who I thoroughly enjoy working with every single day of the week. I wake up every morning excited to go to work which is a rarity this day an age. Over the past 4 1/2 years I have met lots of patients who have walked into my office looking for dental care. Whether it was tooth whitening to a root canal to Invisalign treatment I have learned a lot from the patients I have treated. I am very thankful for the opportunity they have given me. Patients come and go unfortunately but the lessons learned will last a lifetime and believe me I learn a lot from my patients. If someone asked me where I thought I would be in 4 1/2 years it certainly would not be anywhere where I have currently positioned my life and my practice. The last few weeks have brought some sad news when we learned that some patients have become ill or worse passed away, a couple of friends from childhood are critically ill and for someone who is 38 like myself you start to look at things a little differently than you did before. I don't know why it takes circumstances like that to make you look at things differently but it does. For everyone involved in my life, my amazing family, wonderful team, great friends, patients past and present thank you for all that you have done and all the you do. I truly believe I am one of the luckiest people alive. I look forward to world ahead of me.

Replacing Missing Teeth - What Is Your Best Option?

For one reason or another, sometimes it is necessary to replace a single or several missing teeth. There are many reasons that someone may need to do this. Sometimes a patient will experience trauma to a tooth due to a fall or an accident. Over time, a tooth may discolor, get an infection, become loose, or possibly need to be taken out right away. Just because you are missing a tooth though, doesn’t mean that you cannot do something about it. In fact, you actually have several options based off of what you are looking for, your budget, or any other factor you may think of.
There is something called the “Good, Better, Best” way of thinking. So if you are missing a single tooth or several teeth, there will be a good way, a better way, and a best way of handling the situation. Let’s take a look at the “Good, Better, Best” way of handling a missing tooth.
The good way of handling missing teeth would be a partial or a complete denture. This will fill in the spaces, making sure that the teeth around the missing one(s) will not shift. This leaves the door open to other options down the line so if you decide that a denture is not your permanent solution, you will not have to worry about moving teeth when you are ready to move towards the next step. Dentures are not made like they used to be. Most dentures no longer need metal clasps and they are much more comfortable than they used to be. In addition, dentures are now more lightweight and natural looking. The downside to having a denture is that most people do not like having a removable appliance. A denture requires a good deal of care and attention, plus there is always the possibility of it getting lost. Dentures are the most inexpensive way to replace missing teeth, but keep in mind that most dentures last about seven to ten years, and then you will need to purchase a new one.
The better way of replacing missing teeth is a bridge. A bridge is essentially two crowned teeth connected to a fake space filler tooth (called a pontic). Bridges not removable, so patients like that it’s not something they need to worry about losing or falling out. Most bridges last anywhere from 10-15 years as long as you are going for your standard cleanings and the bridge is maintained correctly. The downside to a bridge is that if you are missing a single tooth, and the teeth beside that missing space are healthy, those teeth still must be reshaped and fitted for crowns. Cost wise, a bridge is more expensive than having a denture fitted, but it should last longer than a denture, as well as look more natural with the rest of the teeth.
Finally, the best option. When it comes to replacing missing teeth, the best option by far is doing an implant. An implant is a fixed prosthetic that rests in the gums, and is implanted into the bone in your jaw.

Most patients tend to go for dental implants because their chance of failing is slim to none, they can last a lifetime, and only one tooth is affected by the procedure, and that is the one that you are replacing. This means that all the teeth around the space remain untouched, while the implant that is being done can be contoured to match the surrounding teeth once it is time for a crown, or final restoration. Implants are the most expensive way to replace a missing teeth, but there are plenty of payment plans available to fit a patient’s budget.
If you are missing a single tooth or several teeth, the most imporant thing you need to realize is that you have options. Many patients feel confused or overwhelmed when going to see a dentist because they do not realize or know which is the best option for them. Life span may be as wide as your smile

People who smile a lot are usually happier, have more stable personalities, more stable marriages, better cognitive skills and better interpersonal skills, according to research. Science has just uncovered another benefit of a happy face. People who have big smiles live longer.

Researchers at Wayne State University used information from the Baseball Register to look at photos of 230 players who began their careers in professional baseball before 1950. The players' photos were enlarged, and a rating of their smile intensity was made (big smile, no smile, partial smile). The players' smile ratings were compared with data from deaths that occurred from 2006 through 2009. The researchers then corrected their analysis to account for other factors associated with longevity, such as body mass index, career length, career precocity and college attendance.

For those players who had died, the researchers found longevity ranged from an average of 72.9 years for players with no smiles (63 players) to 75 years for players with partial smiles (64 players) to 79.9 years for players with big smiles (23 players).

This isn't a bunch of psycho-hooey, the authors said. Smiles reflect positive emotion. Positive emotion has been linked to both physical and mental well-being. They added a caveat to their study: "The data source provided no information as to whether expressions were spontaneous or in response to a photographer's request to smile." Still, big smiles are more likely to reflect true happiness than partial smiles.

Maybe the non-smilers were thinking about batting averages.

The study is published in the journal Psychological Science.

Implants Restore Confidence in Denture Wears, Says Philadelphia Dentist Dr. Leonard Tau

PHILADELPHIA, PA – Losing teeth shouldn’t mean losing self-confidence and the ability to eat certain foods.
And yet, that is what happens to many Americans when they lose their teeth due to trauma, periodontal disease or decay and replace them with traditional dentures.
Although dentures can provide an natural-looking alternative for missing teeth, wearers often complain about their difficulty in eating, speaking and even smiling while wearing them, said Philadelphia Dentist, Dr. Leonard Tau. But it doesn’t have to be that way.
For years, Tau, a Philadelphia Dentist, has used dental implants to provide patients with a better alternative to traditional dentures that allows a person to smile, speak, eat and even kiss with confidence. Known as implant supported overdentures, these dentures use precise dental attachments to hold the denture in place and prevent it from slipping.
The process of getting implant supported overdentures often can be completed in one visit and begins with placing the dental implants. In the upper jaw, between two and six implants may need to be placed to achieve an optimum, secure fit. When it comes to the lower jaw, two implants may suffice, provided the patient’s bone height and shape of their jaw are favorable to support an overdenture.
These implants adhere to the jawbone over time through a process known as osseointegration. The implants create an anchor and are used to lock the overdentures into place.

“Dental Implants absolutely can change the life of a denture wearer for the better, particularly when we’re talking about lower dentures,” said the dental expert. “Because the lower jaw is moveable, it is difficult to retain a traditional lower denture. They rarely fit well, no matter how well-made they are.”
Traditional denture wearers often can’t chew food without their dentures moving around. Implant supported overdentures make the everyday things possible that many people take for granted, Tau said. He recently treated an 80-year-old woman and when he spoke with her the next day and asked what she had eaten, she replied “I had a cheesesteak- something I hadn’t eaten in years!”
Traditional dentures can do more than rob people of their self-confidence, they can alter the physiology of a person’s mouth, says Tau. He described the effect of traditional dentures that sit on the gum and bone as being similar to placing a weight on your arm and leaving it there for years.
“The shape of your arm where that weight sat would change as a result of pressurization,” he said. “Similarly, the constant pressure resorption of the jaw and tissue placed by dentures actually deteriorates the jaw bone over the years.”
As these changes occur over time, dentures begin to fit less snugly and securely than when first made, Tau said. This doesn’t occur with implant supported overdentures, because the implants provide stability to hold the dentures securely in place and preserve the surrounding jaw bone and tissue from resorption.
Implant supported overdentures also prevent the need of using denture pastes and adhesives.
“Those can get nasty as far as odor, smell and bacteria,Tau said.
In addition to restoring the normal abilities discussed earlier with implant supported overdentures on the lower teeth, using them on the upper teeth carries great benefits as well.
“Using these dentures to replace the upper teeth uncovers a person’s taste buds and can make eating more enjoyable again,” he said.
Traditional dentures feature a portion of plastic that covers the roof of the mouth, which is where the taste buds are located. People who have replaced their traditional dentures routinely report that in addition to being able to eat a wider variety of food and chew it adequately for proper digestion, they can taste their food better.

Dental Insurance — Is It Worth It

More often then not, dental insurance is a huge factor when it comes to one deciding if they would like to have their dental treatment done or not. However, until it is time, most people are completely unaware what their dental insurance covers, what it doesn’t, and to some extent — some are unclear on who their dental insurance carrier even is! This not uncommon at all. I’m sure we can all remember signing up with our employer’s dental plan, receiving a huge packet of information, and setting it right on the dining room table. Not exactly something one feels like picking up and reading on a day off.

If you were to open up that booklet, what’s inside is an enormous list of dental procedures. It lists what the procedure is and the percentage of that procedure that your plan will cover. When patients come into our office, one of the most commonly used phrases about insurance is: “I don’t understand what kind of coverage I have.” This, in our opinion, is exactly how the insurance company likes it.

The first misconception about dental insurance is that it is just like medical coverage. With medical coverage, when something is wrong, you go to the doctor or the hospital, and everything is normally taken care of. This is not the case with dental insurance at all. What most don’t realize is that every dental insurance has what they call an “Annual Maximum.” An annual maximum is the amount of coverage your policy will cover a year. Most dental insurances allow an annual maximum of about $1,000-$1,500. These annual maximums have not been changed now for about 20-30 years. Obviously, the cost of dental treatment has gone up from 20 years ago, so realistically, this does not cover much if one needs more than a cleaning or a cavity. The typical cost for major work, like a crown (cap) could range anywhere from $900-$1,300. That, right there, would drain most of your benefits for the year, and most are unaware that this annual maximum is even in place!

Percentages are another tactic that insurance companies use to make it look like they are paying for most of your work. However, what they typically don’t tell you is that these percentages are based off of what is call “Customary, Usual and Reasonable Fees.” This are known as UCR fees. What this means is that a dental insurance company has a set fee for every procedure. Again, these fees have not been updated for many years, so while dental offices continue to raise their fees as the years go by, dental insurances still only cover the same amount they did over 10 years ago. Funny how that works, isn’t it?

In and out of network is another issue that can be confusing at times. One often wonders: “What’s the difference?” Well, there isn’t really much of a difference. If your dental insurance is a PPO, then like medical insurance, it means that you can see whichever provider you choose — there is no list. However, if your insurance is an HMO or a DMO, then there is a specific list that you must choose from in order to have your dental work covered. That being said, there are many things that HMO and DMO plans do not cover, some of which are very basic procedures. A PPO plan is a little bit better, but again, there are still some relatively basic procedures that are not covered by these as well.

Let’s go back to UCR fees though, since this is a very hot topic. When an office is considered “In Network” with your insurance, they have been contracted to charge a certain cost for a procedure. This means that they have decided that they are going to chage the UCR fee according to your insurance, so they are essentially charging you what it would have cost to get the same procedure done over 10 years ago. Out of network offices are allowed to charge what the procedure costs today. However, the insurance will only reimburse based off of their UCR fee.

For example, let’s say that a cavity costs $150. Insurance says that they will cover 80% of a cavity based off of their UCR fee, which is $100. So you will receive $80 as a reimbursement. This is actually very generous of an insurance company, since UCR fees are not normally that high. That $80 is deducted from your annual maximum, so imagine if you need major work like a crown, bridge, or even dentures. The even worse thing about dental insurance is that 99.9% of them say that if you are missing a tooth prior to having their insurance, they will not cover replacing that tooth/teeth.

So there are a ton of questions that you should ask when you are thinking about signing up for dental insurance. First, ask what the annual maximum for the plan is. Second, figure out if they have a “Missing Tooth Clause” in case you are thinking of replacing teeth that you are missing. Finally, really sit down and think about if it is even worth it for you, financially, to get dental insurance. Most times, when someone needs work done, they end up paying more for their coverage than for what they actually get back from it. Also, many dental offices, including the Pennsylvania Center For Dental Excellence, offer several different payment plans to make dental work affordable, some of which are even interest free! So for those things that insurance will not cover, or when you run out of your annual maximum, there are still options at your fingertips to get your work done!

The most important thing is that you do not let your insurance dictate the care you should receive. If you need something, you NEED it. A doctor knows more than an insurance company. Holding off on the small things your insurance doesn’t cover only leads to large things that they also won’t cover.

To all our office friends and family,

As we approach the end of 2009 and the beginning of a new decade I wanted to keep everyone updated about exciting things related to your dental office. Over the past few years I have consistently strived to offer the very best in patient care and comfort to my patients here at the Pennsylvania Center for Dental Excellence. I can assure you that this will continue for years to come. I will occasionally be updating everyone to let them know about new and exciting things here at PCDE.
I would like to take this opportunity to wish everyone and happy and healthy holiday season, with healthy being one of the themes of this newsletter. Oral cancer kills one person every hour every day and I recently acquired a state of the art technology that allows for very early detection of this disease. The Identafi 3000 uses uses three different wavelengths of light to excite oral tissue in distinct and unique ways which identifies abnormal tissue with more accuracy than a single color wavelength. This three wavelength optical illumination system allows me to catch early cancers that are not visible to the naked eye. During your visit your hygienist will discuss the advantages of this new detection method. I implore all of you to take advantage of this technology.
Another new technology that I just instituted recently is a soft tissue diode laser. The Picasso laser allows me to expand the treatment options available to you. It will assist me in a wide range of dental procedures that I normally would have to refer out to other dental specialist. For more information about this new technology ask one of my team members.
For those who may not be aware we have started using an online portal for patient confirmations and other communications via text messaging and email. We have found this to be very helpful in cutting down on cancellations and missed appointments. If you have not done so already we would appreciate if if you let us know your email address or if you change it please let us know as well. This program also allows you to request an appointment online as well as leave feedback about your experience during your visit to our office. This feedback actually gets posted to various websites and allows potential patients looking for a new dentist to read about your experiences at PCDE. If you would prefer us to still confirm your appointments via the telephone be sure to let us know.
With the advent of social media (ie Facebook and Twitter) I have created both a Facebook “fan” page and a Twitter feed that will also keep you updated as to things occurring in the office. At various times of the year we run specials on services such as bleaching as well as contests and I do not want you to miss out on these great opportunities.
